The Future of Serious Reportable Events (SREs)

November 12 at 12:00 pm - 1:00 pm

This session will examine the evolution of the National Quality Forum (NQF) Serious Reportable Events (SRE) framework from its last update in 2011 to the comprehensive 2025 modernization. Participants will explore how significant changes in healthcare delivery, including the growth of ambulatory care, telehealth, and increasingly complex care environments, coupled with lessons learned from the COVID-19 pandemic, demonstrated the need for a more current and consistent approach to patient safety event reporting.

The presentation will also review the alignment of the updated NQF SRE List with the Joint Commission Sentinel Event framework and discuss the risk management implications of moving from parallel reporting systems to a unified national standard. Attendees will gain insight into how standardization can reduce duplicative reporting requirements, improve data consistency, enhance organizational learning, and support more meaningful trending and benchmarking across healthcare settings.

Finally, the session will highlight practical opportunities for risk, quality, and patient safety professionals to leverage the updated framework to streamline processes, reduce administrative burden, and strengthen enterprise risk management efforts while maintaining a focus on identifying, analyzing, and preventing serious patient harm events.

Learning Objectives:

Upon completion of this session, participants will:

  1. Understand why the NQF Serious Reportable Events List was updated after 15 years and how the 2025 revisions reflect today’s healthcare environment.
  2. Evaluate the benefits of aligning the NQF SRE and Joint Commission Sentinel Event frameworks to reduce reporting burden and improve consistency.
  3. Identify opportunities to strengthen risk management through streamlined reporting, standardized data, and more efficient safety processes.
