THE ORGANIZATION

Community Memorial Healthcare (CMH) is a nonprofit health system, created to serve the healthcare needs of Ventura County. The organization consists of Community Memorial Hospital – Ventura (263-beds) and Community Memorial Hospital – Ojai (25 bed critical access), CMH Continuing Care Center (75 bed SNF), plus 23 Family Health Centers (clinics) providing primary & specialty care services throughout Ventura County. CMH also operates a number of residency training programs accommodating 80+ newly graduated physicians. CMH provides their med/mal insurance. Programs include: Family Medicine, Internal Medicine, General Surgery, Orthopedic Surgery and Psychiatry residencies. CMH has achieved CMS 5 STAR status, Healthgrades award for Patient Experience – one of the top 15% of hospitals in the US, and US News & World Report award for Patient Experience.

THE POSITION

The Director Patient Safety & Clinical Risk Management is responsible for monitoring, planning and implementing the Patient Safety Program across the health system. Responsibilities include, but are not limited to, managing and analyzing event data (RL Datix); conducting educational programs; directing service recovery; leading investigations and root cause analysis; conducting the Culture of Safety Survey and creating an action plan to address results; providing direction and support for disclosures and CANDOR (early resolutions); complying with risk management-related standards of accrediting and regulatory agencies with the objective of enhancing patient safety; promoting high quality care and minimizing loss. This individual develops policy and/or organizational changes. Will supervise a Manager/Clinical Risk & Patient Safety and a Patient Experience Coordinator. Reports to the VP/Quality & Risk.

DESIRED KNOWLEDGE, SKILLS & EXPERIENCE

• Ability to embrace and promote the Mission and Values of Community Memorial Healthcare
• Bachelor’s Degree required (BSN or Clinical Field), plus JD or Master’s & current RN also preferred
• Previous success developing, managing & improving Risk & Patient Safety programs in acute care settings
• In-depth knowledge of Clinical Risk Management, Patient Safety, Patient Experience & Professional Liability
• Working knowledge of Just Culture, CANDOR (healthcare) & TeamSTEPPS, as well as Regulatory Compliance
• Data-driven leader with robust skills in collection, analysis and interpretation for loss prevention & mitigation
• Motivating communication style and success with diverse, multi-disciplinary personnel, including physicians
• Broad understanding of Hospital & Medical Group Operations
• Strong RCA experience, as well as outstanding report writing, presentation skills and teaching ability
• Excellent interpersonal abilities, customer service skills and public relations savvy
• A sense of humor
